他皮膚發黃苦喊上腹痛 醫檢查驚現2種癌
一名40多歲王男(化名)日前因上腹疼痛就醫，醫師發現王男有黃疸症狀，電腦斷層檢查顯示，左肝有浸潤性的腫瘤合併肝內膽管擴張，醫師幫王男轉診至台大醫院新竹分院，接受進一步檢查，確診為罕見的合併肝細胞癌及膽管癌，經肝門靜脈栓塞及化療後切除。
收治該病例的台大醫院新竹分院外科部一般外科主任醫師黃俊傑表示，原本計畫直接手術切除，但病人同時被檢查出有慢性B型肝炎且肝功能不佳，無法負荷左肝切除。經癌症多專科團隊討論後，先由放射科醫師執行左肝門靜脈栓塞，再由腫瘤科醫師安排化學治療，同時請胃腸肝膽科醫師開立B肝藥物治療。
經過兩個月的治療後，再次安排檢查，發現腫瘤無明顯惡化且肝臟殘餘功能恰好可負荷左肝切除，才交由肝膽腸胃外科醫師進行左肝切除合併廣泛淋巴節廓清手術，術後第6天順利出院。切除的腫瘤經病理診斷為罕見的合併肝細胞癌及膽管癌，病人術後不必再接受化學治療，但仍需持續追蹤。
黃俊傑表示，肝癌目前為國內十大癌症死因第二位，發生率第四位，而原發性肝癌中最常見為肝細胞癌，其次則是肝內膽管癌，主要治療方式均為手術切除。但須視病人狀況而定。
黃俊傑解釋，肝細胞癌主要的危險因子為病毒性肝炎及肝硬化，發生時通常沒有症狀，膽管癌則可能與先天膽道異常、慢性膽管發炎或感染等有關，若發生在肝內通常也沒有症狀或僅有疼痛及黃疸，且只有早期發現才能有效治療。
因此黃俊傑呼籲，若民眾有慢性肝炎、肝硬化，一定要找胃腸肝膽科醫師定期追蹤，若發現有黃疸症狀也要立即就醫，檢查發現有肝臟腫瘤，再省錢大作戰熱銷商品找肝膽腸胃外科醫師評估是否可手術切除。
